Turning a blind eye to the far field: Are we burying the evidence? A case of abrupt catastrophic implantable cardioverter defibrillator lead failure causing sudden death

چکیده
Introduction Implantable cardioverter defibrillator (ICD) therapy is commonly used for the prevention of sudden arrhythmic death in high risk populations. A recognized complication of such therapy is lead failure, leading to potential inappropriate detection and delivery of high energy defibrillation shocks. Many modern devices utilize algorithms which can differentiate between lead noise and true arrhythmia, avoiding unnecessary therapies. However, the programming of these algorithms can be the difference between life and death. Below is presented a case of lead failure in which the default programming of the device’s algorithms contributed to a catastrophic outcome which was potentially avoidable.

Background & Aims: Nowadays, the quality of life and how coping and treatment of patients with an implantable cardiac defibrillator has led to concern and attempts to reduce or eliminate the problems of these patients in the people who provide health care services. This study aimed to determine the effect of nurse-peer-led support intervention on quality of life in patients with an implantable ...
متن کاملUndetected ventricular fibrillation in a single-chamber implantable cardioverter-defibrillator: When the far-field channel sees more than the intraventricular channel
Introduction Implantable cardioverter-defibrillators (ICDs) represent the treatment of choice for the prevention of sudden cardiac death. Low signal amplitudes are occasionally encountered during placement of ICDs. ICD implant guidelines state that the amplitude of the sinus rhythm R-wave recorded from the ventricular electrogram should be higher than 5 mV. If no sufficient R signal amplitude c...
متن کاملImplantable cardioverter-defibrillator in a patient with dextrocardia situs inversus
Background: Dextrocardia is a congenital anomaly, which may have coexistent coronary artery disease (CAD), arrhythmias and conventional indications for device therapy. However, the implantation of transvenous leads can be technically challenging and the approach needs to be tailored to the patient's individual anatomy. Case presentation: A 54-year-old male with dextrocardia situs inversus and i...
متن کاملHome monitoring report from a single lead Lumax DX implantable cardioverter defibrillator: New observations in a new system
A 56-year-old man underwent a single lead Lumax 640 DX implantable cardioverter defibrillator implantation for primary prevention of sudden cardiac death. A DX system consists of a single lead, which provides atrial as well as ventricular electrograms, and enhances atrial arrhythmia detection. Three months after the implantation, high-frequency episodes were detected on the far field and the at...
